Not a builder... This may be obvious
On interior walls of an ICF home, do you normally just fasten sheetrock to the foam block, or should I plan on framing in the walls that are backed by the ICF foam? 

What about bathrooms or any place where you have a wall that is not just painted... such as tile or wood paneling.

Right now I am just planning, drawing floor plans etc and am trying to determine how thick to scale the exterior walls

The more I think of this, it seems that I would need to build a 2x4 home inside the ICF.

There is no need to build stud wall s inside an icf wall. Most icf blocks have studs built into them on 6-8 or 12" centers. You fasten your sheet rock directly to them. Your wiring goes in the foam so for most applications you should never have to build a stud wall. Go to a few icf websites and you can view a cutaway.

Take a look at the first thread titled "ICF Learning Center". Shouldn't take long to find an answer to your question and there are even a few drawings.

Like GeoDean's summary of info sources for Geothermal, this is a good place to start for ICF newbies. Then learn how to search all the threads for specific topics and more detail.
