insulation ideas for mild climate (VA)

I have read seveal comments for northern and extreme southern areas but now need help with VA new construction. Stick built 2X6 external wlls and poured foundation. Roof will be 12:12 pitch-no catherdial ceiling took that off to make 12ft ceiling.
outside walls: Thinking of adding 1-2" ridgid fom board sheathing on outside before stucco. This would be for reducing effect of the thermal bridging of the studs. Add 2 1/2" close cell foam in 2X6" followed by blown in cellulose (dense packed).
attic: (unconditioned space) Not sure if I really need to spray foam on top of trusses or just insulate floor with blown in cellulose or Icynene. If I want at least an R-40+ for attic maybe 2 1/2" foam (closed cell) followed by blow in as stated above for walls. What about radiant barrier in attic with all this insulation, is it still needed? I also keep reading the pros and cons on fan circulation. Not sure that is a real benefit. Just trying to be smart in using a combination to get the max benefit and keep costs down.
My main floor is 2300 sq ft below attic. Also have read it is a good idea to foam seal and insulate foundation sill. As a side note I would be interested in hearing about insulating the floor joists against noise as well as insulating. I have used I-joist before and now looking at web joists for running my ducting through but know they carry noise more. I hate to hear the feet about me. This house will have a main level and a finished walk out basement level. (also 2300 sqft). Any ideas/experience on my situation will be most appreciated.

1" foam on the outside + 1-2" of spray foam + cellulose is what I would use for walls. Spray foam + cellulose for the ceiling.
