Is there any best practice with regards to installing a lineal drain at the time of pour? How do I put the slope in for a curbless 3' by 5' shower?

Well if the CC guy is really good, he could probably shape it by hand if you are able to locate and orient the area properly around the drain, just like they typically would with a floor drain. YOu would of course have to plan out the vertical recess to allow for the tile work.

It might be easier to block out the area for the shower a little deeper than necessary with a wooden form and build-up/grout in the shower later to the desired height.

Me too, it's done with a damp mixture of portland/ sand - dry packed and shaped with trowel

I quit using mud pans several years ago and switched to Shuter or A knockoff - Prosource. Also use the membranes on the walls instead of mud. More expensive, but a huge timesaver.
