Walkout basement door sill detail

I'm getting ready to pour my basement floor slab. I'm trying to figure out the right detail for the 36" walkout door. The top of the floor slab will be about 4" above the current rough opening.
Where should I end the slab - flush with the exterior wall, flush with the interior wall or somewhere in between?
I'd sure like to have a thermal break between the slab and the cold winter weather - what is the best way to do this?

We usually end our slabs at the inside of the foam on the outer layer of eps on the icf. This provides a nice thermal break from the outside temps. We then install a piece of sheet metal (bent at 90 degrees) over the foam to support the sill of the door.

Greg,
Peter has a great detail there. We use a very similar method with exception that our metal at the bottom is a pan of stainless steel. The reason we use stainless is, in a daylight basement your chance of water exposure increases greatly.
