My ICF door openings have the concrete between the forms exposed under where the door threshold will sit (the doors will be mounted toward the inside of the wall). It seems like there will be some thermal loss from this point. What do people do to cover this up? Do you put 1/2" of insulation over the top of the concrete before installing the door threshold? I'm wondering if that would create a stable enough base? Thanks.

We always allow for 1 1/2" pc of treated lumber under the doors at least If there is sub floor we allow for that also. I think it makes a cleaner job.

Look at these threads to see what I did back in 2008.

I poured a concrete sill that is tied into the wall concrete. There is an insulating space between the sill and subfloor. The thresholds are ADA compliant made with a thermal break under the door. There is a thermal connection between the exterior and the wall concrete so obviously there will be some heat transfer take place between outdoor conditions and the wall concrete. But, you can't stop every bit of transfer and figured in the grand scheme of the whole house it wouldn't be enough to get wrapped around the axle about.
