Jess Where can I find rebar placement for window and door headers. Simple house single story slab on grade. Or an engineer in Washington state Jess

Where in Washington state are you looking at, or more specifically what seismic zone in Washington state? The specs in the code referenced by New Boston are limited to seismic zone A thru C(second paragraph limitations). For seismic zone D(lots of those areas in WA) you need to design to PCA-100 or ACI 318 or get an engineers signoff. You can get free access to PCA-100 at the Portland Cement Association website at http://members.cement.org/EBiz55/Bookstore/BookstoreHome.aspx PCA-100 will have prescriptive specs for opening/lintel rebar requirements for seismic zone D1 and D2.
