I do all my floors monolithic in Idaho. 1 pour and I have finished floor. We Devoloped a eps stay in place form specific to mono slabs. Insulates to r-20 and eliminates kickers and return trip to backfill form after pour is complete.

Mono slab and footings is fantastic and the way to go if you can get plumbers to play with your schedule.
We dap out the edge of the slab 1.5 inches for water and shear principles.
We post tension the slab because it is cost effective and gives a crack free floor.

irnvek, do you have any vids or pics of that application? we do that on shallow application footings, but need pics for the post tension ?

Take a look at Branch River Plastic's product for monolithic footer, wall and slab pour, it's similar to what we do. It's pretty slick.
