I would like to know what is the cost ratio is to build with ICF within Ontario, Canada vs. standard wood-frame construction and is there any truth the you can expect to pay more for typical things ie, windows and doors because of the added thickness of the ICF form.

It is going to depend a lot on the design, and the installer in Ontario. Location will also have a significant impact. For instance, in southern ontario, the cost will be higher than in Northern ontario, simply because of the insulation, vapour barrier and air barrier requirements being much more stringent in the northern 2/3.
