Here's one for those in the know. My poured slab will be 22" above the footing on fill within the completed ICF system. I'm on fairly wet lowland. Should I waterproof the inside foam as well as the outside below grade?

YES. Water will wick up the wall otherwise. You may want to seal the construction joint between the slab and wall as well. You may also want to install a drain field under the slab that is tied to a sump well and pump if the potential for extreme amounts of water is possible.
