If one has a slab-on-grade home design and will be installing a concrete patio that will transition from the two 1st floor entrances of the home. How does one thermally break the threshold transition area that goes from the interior to the exterior patio? I know they sell aluminum thresholds that are thermally broken but I need to know how the thermally break the CONCRETE.

Most architects just detail it as ONE large monolithic slab pour from the interior to the exterior but that creates a thermal bridge. Anyone have a detail they can share for this condition?

You just insert a piece of 2" XPS EPS in there before the next pour.  In locales with more extreme temperature differences, you will need a larger space, of course.

And you can cover the rigid foam insulation with a 1x2 (if using 1" thick foam) or 1x3 or 1x4 (if using 2" thick foam) PVC trim board such that the top of the PVC trim is flush with the top of the concrete. This will protect the foam from physical damage. You can buy this material at Lowes or Home Depot.

Any architectural details you can upload or link?

Would the EPS thermal break take place right at the threshold area or just below the step down where the concrete patio meets the interior slab?

Can this thermal break be placed under/at the threshold door frame to hide and protect it?
