soooo the fellow has zero room for insulation on top, door heights and headers in place... 2 inch slab topping pour w/ pipe, good thing is pipe is in most upper portion of slab, preferred by RPA guide lines.
The bottom of the plank is in conditioned space with height restrictions, I again would suggest 1 inch spray foam and sheet rock, R-6 or air gap 1 inch and Bubble foil bubble, R better than nothing....

I will advise the customer to consider ceiling insulation.

1 inch of any real insulation would be a lot better than nothing.

- Check with your structural engineer before you put insulation under the topping layer on Spancrete. Sometimes the 2-3" topping is counted as part of the concrete design. It's probably won't affect you, but it's worth checking on first.

- Using rebar in place of wire mesh in might not meet ACI standard for minimum concrete cover distance.

- Regarding the high thermal mass, we had some good discussions here: http://www.greenbuildingtalk.com/Forums/tabid/53/forumid/12/postid/70675/view/topic/Default.aspx

Hope that helps...

The hollow core plank manufacture recommended a vapor barrier between their plank the topping.
